I hereby agree to and abide by the rules & regulations of UUPTWA as come into effect from time to time and pay all the dues as applicable from time to time. I also agree that my membership may be terminated immediately if the UUPTWA Governing Body concludes that I have violated organization’s rules & regulations, instructions & failed to maintain the decorum.
